10/15/2023 EnchantaVaughn Approved Behavior: Client was in a passive mood, as evidenced by his easy-going expressions. Client explained that he wanted to have session without mom present so that he could get a chance to show his improvements in the home. Client was granted permission to have the session in his room as long as it was clean, and he kept the door open. Client is still adjusting to the changes in the home and giving his mom space for self-care and to rest. Client said he wanted to have one on one with his counselor and figure out some things that he has been having trouble with understanding. Client was respectful of others in the home. Client appeared to be gaining a lot of new skills when he allowed himself to learn from his mistakes. Behavior: Client shared positive and negative thoughts concerning his expectations and responsibilities and how he feels overwhelmed at times. Client explained his frustrations and why he has been more stressed than normal lately. Client wants to be able to balance everything and still be a kid. Client expresses that he misses having all the attention but understands that he has to be the leader. Client was apologetic for the way that he responded to what his mother was saying. Client shared that he will work on building a better relationship with his mother and father, if his parents would let him do more for himself. The client continues to present sadness whenever he has to talk about his feelings involving both of his parents and who he wanted to live with. Client appreciates getting the help that he needs and having someone he can talk to about everything. Intervention: QMHP processed client positive and negative behaviors in the home. QMHP assisted client in developing more social skills around his family. QMHP inquired about possible challenges client can experience with applying this skill. QMHP provided positive feedback. QMHP applauded client for making the big decision to want to work on changing his behavior. QMHP discussed with him the importance of working hard to make sure that he continues to stay focused on his goals. QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve his goals and objectives. QMHP provided praise when the client continued to display positive behaviors throughout the day, remaining on task and was very respectful to all adult figures. Intervention: QMHP isolated client from distractions to decrease disruptive behaviors and to reduce excessive talking. QMHP guided client by asking him why he believed his family was important to him and what is the overall image that he wants to depict when it comes to his family. QMHP assisted client in creating goals for his family and understanding what role would be in achieving them. QMHP role played with client and demonstrated for positive behaviors and responses to increase positive interactions between him and the family. QMHP applauded the behavior of client and recognized how mother and client had been interacting positively. QMHP reviewed an article with the two that discussed relationship skill building and ways to better a relationship with one another. QMHP engaged in conversation with client about positive decisions and his plan to continue building relationships with his mom and loved ones. Response: Client did well with processing his feelings about positive and negative behaviors in the home. Client agreed to be more aware and patient with being there for his family and cherish having a close family. Client realized he could be a little more supportive and cheer on his family rather than fighting them. The client responded in a constructive manner acknowledging the affirmative reinforcement that the QMHP provided with a warm smile and head nod. Client utilized relaxation strategies appropriately. The client showed his gratitude for being able to release physical tension stating that he felt relaxed. Client was able to cooperate and comply to rules and regulations due to positive reinforcement provided by the QMHP. Client implemented pro social skills successfully. Response: Client has presented himself in a responsible way and is making progress towards his goals and objectives. Client was able to utilize coping skills to eliminate stressors and unwanted feelings. Client displayed self determination to stay focus and pay attention. Client utilized meditation and relaxation techniques. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques. Client verbalized positive feelings about building relationships in the home, despite him feeling alone and unloved. Client is making progress with expressing his feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation, and waiting for his turn to talk before blurting out loud. Client was able to speak calmly and demonstrate his improved communication skills while engaging in self-reflection.
